Help with FET Driver HIP4080A

Hello,

I'm eventually trying to Drive a small motor with the HIP4080A, but for thee time being I'm just familiarizing myself with the chip.

Here's the Datasheet:

To get started, I'm just trying to put some basic inputs and see that the basic outputs are operating as I'd expect.

So I did
5V = HEN, IN+
GND = VSS, IN-, DIS,
12V = VCC, VDD

Based on the truth table, I'd expect to see see AHO and BLO with 12V on them coming out, but I'm also seeing 12V on BHO too. This is causing the left side of the bridge to short (And me to burn my finger on the transistor I had hooked up!)

Can anyone help me with why this is happening? Am I doing something wrong?

Well, I don't know that anyone even read this, but in case someone does in the future, I think I found my problem. I did two things that appear to have worked.
1. I added the Bootstrap Capacitors and Diodes. I think that there might have been some feedback required on those pins to get the chip out of the startup sequence.
2. The big problem is that there was no way for the charge to dissipate off the gates once I removed the "On" signal. By adding a pulldown resistor (I used 4700 Ohms, but probably I should make it bigger) to each of the gate drive pins (AHO, BLO, ALO, BHO) the gates turned off when they were supposed to.
