    What are each of the relay contacts connected to ? Some supply voltage that is not present in one ? And to a board supply rail the other contact ? Have you checked surface mount fuses for continuity ? Do not risk circuitry by jumping traces unless knowing what is being done.

    Poor connection made when flexing the PCB

    I would attach the board edge? to a bench vise, a meter probe to perhaps soldered to its ground and the other probe at many places with one hand while the other hand deforms the board to get near the problem trace or solder. Yes, found open vias.

    Are you sure the LEDs are 5V ? Try first to power the 4 LEDs in series to 12V capable of 2 Amperes, and come back. 12V(+)-------|>|------|>|------|>|------|>|---------(-) If you want to run all 4 LEDs from a 5V supply, the 5V supply would have to be capable of 11 Amperes, not 2.1A
