@Numnuts , As the rectifier and buffer caps are on the board, you will need a dual AC transformer as input:
ILP has a series of transformers that can be used.
Use a high enough power rating for the used amplifier.

It looks like the snubber circuit is not in a good shape.
The resistor lloks burned and I can not tell you things about the status of the capacitor.

When you look at the 4558 datasheet, you will see that it has an open loop gain of 100 dB.
Each 20 dB is a factor of 10, so 100 dB is a factor of 100.000.
Have a look at the attached PDF for more info on the decibels.
