    If the meter won't work on the volts range (where it doesn't require batteries) it is possible that the meter itself is defective (damaged deflection coil) which is common in older devices such as yours. It is sometimes possible to have the meter repaired but it is a skilled and expensive task.

    Discard any reading made using 'just one probe'. You can only make correct readings when both probes are connected. Is the transformer disconnected from all other sources other than the AC mains input? The readings you are getting do not correspond to the diagram shown in your original post...
