Transistor pair to switch between Voltages

Looks fine. Note that they are P Channel MOSFETs, not PNP MOSFETs. We've also spoken about the resistors for the LEDs. Neither of the issues will stop it working :-)

41 volts in my DVD's AV output jack ground.

The correct way to test a fuse is using the ohms range on your multimeter, with the power to the appliance off (and with it unplugged) and without any resistor. Remember what I asked you to do ages ago?

41 volts in my DVD's AV output jack ground.

That's probably your problem. However we don't know if it was the original problem or not. In any case, to effect a repair, your have to determine what this IC is and replace it. I can't see any margins on the chip other than the index mark which indicates pin 1. Even if you do manage to...

41 volts in my DVD's AV output jack ground.

No that's just flux residue left from the manufacturing process.

41 volts in my DVD's AV output jack ground.

I don't know (there are several possibilities), but the test I've asked you to do will help us establish a few facts which might help narrow down the cause and possibly the problem.

Where to count legs from for TO-94?

You number the legs based on the shape of the package, not where the label is placed (the are unlabeled devices). The best thing to do is to look in the product's datasheet which will typically show all the packages and label or otherwise describe the numbering.

41 volts in my DVD's AV output jack ground.

Was that in reply to me? Nothing is going to get "busted" with the power disconnected.

41 volts in my DVD's AV output jack ground.

Let's start with the basics. And with the power OFF. A general photo of the outside of the unit will help us point you at stuff to look at. Check out the power switch. Does it physically switch the mains (does the mains wiring run up to the switch)? Measure the continuity of the wiring...

Best way to detect what power source is connected?

You've fine to the effort of having "perfect diodes", adding the fuses will put back some of that voltage drop. This is even worse if you're considering polyfuses.
