This is not a breaker, it is a disconnect switch which is not designed to
open under load. Normally, the breakers are operated then the disconnect is
opened. This one appears to have been opened under normal load current.
Proper circuit breakers do their business much faster ( generally under 1/20
second) and with no fireworks.

In any case this shows why oversized knife switches are not used to open
high voltage / high current circuits.
