Help with PWM - SG3524N pinout

Long time no post !

Attached a schematic from a patent that I want to build, can anyone please help me with the pin numbers on the PWM - SG3524N chip for the corresponding connections to the chip as shown, before I start making a PC-board. ???

This is what I have figured out from the datasheet so far, also must some of the other pins not used be tied to ground or positive rail.

Please correct me here.
