    Inition signal in.... ah ok you step it down to 5v Is this a 5v relay? If it's 12, it's only going to see 5v, it might struggle to energize You're not far off ... you need a resistor on the base of the transistor or you'll risk cooking your micro controller, you could also do with smoothing caps

    It's normal for the voltage to drop like that under heavy current draw, but these tshirts run on aaa's so i can't imagine it being more than the adapter So maybe your adapter is faulty or the tshirt is causing a short? Ideally you need to measure the current draw with a multimeter
