I'm not good interpreting datasheet. Are you sure that 4017BD output pin can directly drive LED to lit?
What the part number of your and gate? It seems its output capable of driving LED to lit.
Maybe if you increase VCC supply into 12V will help to lit your LED and remove the current limit...
It's more accurate if you lift the diode from pcb before doing the diode test. Reading of 0.7V in 1 direction and OL in another direction means diode was ok.. Reading of 0v means shorted diode.:)
The small pcb looks like the car plug cigarette adaptor. Just wondering that how come there were big diodes for those small loose wires. I think those wire was for the supply of sd transmitter. Much better if you can locate the broken portion of wire on solder of pcb and post bigger picture on...
Possible a loose connection or cold solder on pcb trace. Use multimeter to test continuity of traces and switch contact.
Can you post the other side of board and showing the push button switch pcb trace.
The reason for the distortion was because of 60hz in ac line ground connection. If you can find wireless or bluetooth audio interface between pc and amplifier it will eliminate the distortion.

PCB traces on picture no.2 were covered by the connector and can't see traces under it.
But if pcb traces 1,2,3 and 4 were connected together and also same with pin 9,10,11 and 12. Then it should work.
You mean pcb etch connector pin1 and pin12 will not be soldered.
Are you sure your circuit does not need these signals on pin1? There's a trace connection on pin1. Your circuit board will not work if there's a signal that part of the circuit on pin1.
