cultural reasons why so many countries have few if any small companies
that do so.

materials enough to build 20 reasonable houses, raising the price of
materials for everyone else. The money he paid to the tradesman he got
by suing doctors (driving up the price of everyone's medical care) and
the tradesmen will use the money to buy stuff, which makes stuff more
expensive for everyone else. Payment without productivity shifts
consumption but creates no more stuff.
